ED Repayment Plan Selection Form

The Department of Education (ED) Repayment Plan Selection form PDF allows borrowers to select an initial payment plan prior to entering repayment or to request a change to a different plan. ED uses this form to collect the information needed to determine eligibility and calculate the monthly payment amount for borrowers who select the Income-Contingent Repayment (ICR) and Income-Based Repayment (IBR) plans.
